Buying the cellular version of an Apple Watch is two purchases, and the second one recurs. The watch needs its own line on a carrier account, and what that costs varies more than most people expect — from nothing at all on one plan to $15 a month plus an activation fee on another.

The spread is wider than the numbers suggest, because it compounds. Fifteen dollars a month against six and a half is a little over a hundred dollars a year, every year, for identical functionality on the same wrist.
The two plans where it is effectively free
Visible+ Pro includes watch service at no additional charge. At $45 a month all-in on Verizon's network with priority data and unlimited hotspot, that makes it the only plan we track where adding a watch costs nothing at all.
US Mobile's $6.50 a month on annual prepay is the cheapest standalone rate, and it pairs with plans that are already among the cheapest ways onto Verizon or T-Mobile. If you are shopping for a phone plan and own a cellular watch, those two plans change the arithmetic more than any watch-line discount will.
That is the useful way round to think about this: the watch line is small enough that it should not drive your carrier choice, but large enough that it is worth knowing before you sign up, because switching later means switching both.
Activation fees are the part that surprises people
Several carriers charge a one-time activation on a watch line, commonly $35 to $40 and in some cases more than three months of the service itself. It is not always quoted alongside the monthly price, and it is occasionally waived in store or during a promotion — worth asking before you complete the setup on the phone.
Family Setup is a separate question
Family Setup lets a watch run without a paired iPhone of its own — the usual reason being a watch for a child or an older relative who does not carry a phone. Not every carrier supports it. Xfinity Mobile, for example, sells watch lines at $10 and does not offer Family Setup at all.
If that is why you are buying the watch, confirm support before the plan price, because a cheap watch line that cannot do the thing you bought it for is not cheap.
For most people the answer is simply to add the line to whatever carrier they already use and accept the $10. For anyone choosing a plan now, Visible+ Pro at $45 all-in with the watch included and US Mobile at $6.50 are the two that make the watch genuinely cheap to run.
